For the consideration of receiving a xx% discount on print prices (receipt of which is hereby acknowledged) in return for modelling for photographs taken by the undersigned photographer, Martin Colbert (“The Photographer”) on xx/xx/2010 at xxx, Ipswich
I the undersigned model (“Model”), hereby grant and agree that the photographs taken of me during this session can be used or published wholly or in part, altered or unaltered, by themselves or with other photographs in:
portfolios, exhibitions, the “Martin Colbert Photography” web site, email, example printed material/products, advertising leaflets & posters, business cards and electronic media such as screen displays, illustrating the products, services or photography of Martin Colbert Photography.
The photographs will not have any wording associated that will be attributed to me personally nor will my name be used in association with the photographs.
I acknowledge by signing this form, subject to any restrictions stipulated above, that I give
up all claims of ownership, income, editorial control and use of the resulting photographs. I acknowledge that the copyright of the photographs belongs to Martin Colbert.
I acknowledge by signing this form that I give Martin Colbert or third parties acting for Martin Colbert authority to publish the photographs in accordance with the terms stated above. I am over 18.
